Type1 bicuspid; low left coronary (dominant); poor CT; unsure of valve size (26vs29) and coronary obstruction risk. Opted for sizing BAV with a 25mm balloon. Thoughts?@JasonFoerst @J_M_McCabe @PradeepYadavMD @PaulMahoneyMD @CathElectroSurg
“Houston we have a problem!” Transcaval impella day 9 waiting for tissue to scar a little more before BASSINET- a few Definity bubbles can be seen in the pericardial fluid. @jasonfoerst@pradeepyadav
Agree that results are suboptimal with muscular VSD devices. We have had some success with BASSINET (hybrid surgical or percutaneous). Requires direct RV access.

I start with this...emergent transcaval 5L Impella insertion and Swan. No planning CT- projection angles and crossing location fluoroscopically identified.
Many still ask if it will reverse the shunt and cause hypoxia. It will not. I find most post infarct VSD shunts ratios close to 3:1 (2.9 in this case). The 5.0 Impella will reduce it to less than 2:1 (1.7 in this case).
