We argue that the view that more trials are needed within for each specific regimen before integrating ctDNA-guided adjuvant therapy withholds benefits of a valuable biomarker (ctDNA) and overtreats many unnecessarily, ignoring compelling regimen-agnostic data. @Yale_Urology
With EV-303/304 & NIAGARA, 'sandwich' regimens in MIBC, @FedGhali & I argue that ctDNA is a marker of molecular residual disease and de-escalation is safe for patients with ctDNA(-) & excellent reponse at cystectomy path. doi.org/10.1111/bju.70377

Glad its approved and have another treatment option! IMO, for 5% improvement in 3y DFS (essentially only recurrence not progression), and associated >15% G3+ systemic AE, this combo does not seem useful to most pts And we have ever growing # of well tolerated salvage tx
Durvalumab now @US_FDA ✅ for Non-Muscle Invasive Bladder cancer based off #POTOMAC: PhIII, BCG + Durvalumab vs. BCG alone in BCG naive HR-NMIBC: - DFS waiting maturity (HR: 0.68) - 3yr mDFS 82% vs 77% - MedOncs will now see HR-NMIBC #gusm #OncTwitter

Huge congratulations to @drtanws on being awarded the prestigious Hunterian Professorship by Royal College of Surgeons of England. A well-deserved recognition of his outstanding work in defining NMIBC risk stratification in bladder cancer. Proud to see you continue to excel and lead on the global stage! @Yale_Urology @UCLDivofSurgery @MDAndersonNews #OncSurgery @RCSnews @SUO_YUO @UroToday

Amid the celebrations, is anyone else concerned that control arm never saw EVP upon progression? Seems like this is another EVP vs no-EVP trial, rather than answering 🗝️ clinical ❓ of early vs late... or am I missing something? @csoncol
In cisplatin-ineligible patients with muscle-invasive bladder cancer, enfortumab vedotin–pembrolizumab plus surgery led to better event-free survival (74.7%, vs. 39.4%) and overall survival (79.7%, vs. 63.1%) than surgery alone at 2 years. Thx @UrologyTimes Often, new Tx's are benchmarked against subotpimal/ outdated controls. We're developing simulation methods to adjust for this & generate more interpretable results. Here, Avelumab benefit was modest— getting the comparison right is essential! @Yale_Urology
WATCH🎥: Fed Ghali, MD, (@FedGhali) highlights results from a simulated analysis of the EV-302 trial that included adjustment for maintenance avelumab in the control arm. @Yale_Urology urologytimes.com/view/fed-gh…

Fellow Uros, w/CREST the relative difference here-32% does not tell the story. In fact 2 year absolute EFS difference is 5%. A 1-2% point swing would be a negative trial like Alban. CIS findings are exploratory based on a few patients & underpowered.
#CREST Trial: Sasanlimab immunotherapy + BCG in BCG-naive #BladderCancer. Sam Chang, MD, MBA @VUMCurology joins @zklaassen_md @GACancerCenter to discuss this three-arm study of over 1,000 patients showing a remarkable 32% reduction in event-free survival events with the combination therapy. The trial demonstrated impressive long-term outcomes, particularly in carcinoma in situ patients where complete responders achieved 92% response rates at three years compared to two-thirds with BCG alone. #FullVideo on UroToday > bit.ly/4mRZQJT

Thx @urotoday ! •EV302 transformed 1st-line mUCC, but had ⬇️use of mAvelumab in control arm pts. •This biases towards EVP-a 🔑concern •We simulated EV302 with mAvel in appropriate pts •EVP still wins w/mAvel in control arm! @Yale_Urology @PGrivasMDPhD @DanielPetrylak
#SUO25 @FedGhali RW simulation of EV-302: How would outcomes change if control-arm chemo pts universally received maintenance avelumab? 🇺🇸 Gated counterfactual simulation used to reconstruct IPD + model adjusted control arm 🔹47.8% of control pts eligible for avelumab 🔹Simulated OS HR: 0.56 (95% CI 0.46–0.70; p<0.001) 📌Even with assumed universal avelumab maintenance, EV+P maintains clear OS superiority → limitation does not materially alter the EV-302 conclusion. @urotoday

Super fun arguing for surgery🗡️ against my friend Joseph Kim at the bulldog debates this weekend. Surgery is safe in experienced hands and can spare many the significant downsides of chemo. Overall great conference run by @Yale_Urology
Replying to @FedGhali
@FedGhali makes the compelling case for primary RPLND for stage 2 seminoma vs. chemotherapy in this @Yale_Urology Bulldogs debate highlight, wins poll vote🔹Chemo toxicity is real; surgical recurrences are salvageable and spare chemo in ~80%. #UroOnc #TestisCancer

Despite what you may hear, single arm NMIBC trials can't be compared due to 1)difference in mandated biopsies 2)allowance of 2nd line therapies after BCG before enrolling 3)differences in eligibility regarding allowance of HGT1 w CIS after induction BCG 4)allowance of reinduction
Congrats to patients and the bladder cancer community on another drug approval in Tar-200. Are we at the last gasp of single arm trials? Bond-003 maturing so maybe CG007. But I think @US_FDA will change their tune and mandate RCTs soon

Totally agree- Consolidation may overtreat some, but many others will benefit. Defining subpopulations & identifying OS-improving interventions in post-EV/Pem pts is a key question. Lucky to present our trial exploring this @Yale_Urology/@YaleCancer , and learn from others
Well said @shilpaonc excellent session by @siadaneshmand & Gopa with great speakers @koshkin85 @FedGhali et al. & awesome patient advocate! We need de-escalation & escalation trials, biomarkers for personalized approach on Tx duration, role of consolidation, etc. @BladderCancerUS
