📚 Previously published in PDI | A pharmacokinetic simulation compared two IP aminoglycoside regimens for peritonitis on APD: both reached >90% optimal peak/MIC and AUC/MIC ratios on days 1 and 5.
#PDI#PeritonealDialysis#ISPDdoi.org/10.1177/089686082312…
Outcomes: time with peritoneal peak/MIC >10, AUC/MIC >74, and plasma Cmin. The loading-dose regimen gave IP exposures 2.5 times greater in the peritoneal compartment on day 1. By day 5, plasma Cmin was similar: 2.5-3.4 vs 2.4-3.3 mg/L (loading vs fixed).
The authors conclude that international guidelines for PD-associated peritonitis can continue to recommend the fixed-dose regimen for those on APD, with plasma Cmin monitoring after 3 days to assess for drug accumulation.

Major themes influencing screening and assessment for home dialysis candidacy: screening tools and guidelines (n=8), relative contraindications (n=4), patient or program education (n=9) and socioeconomic factors (n=2).
The authors conclude that patient screening and assessment for home dialysis requires a comprehensive evaluation of clinical, psychosocial and logistical factors, supported by up-to-date training and education for providers and patients.
