Thrilled to share #PRIMARY2 Trial Results. This is the first randomised trial to show that adding [⁶⁸Ga]Ga-PSMA-11 PET-CT to MRI can safely halve the number of men needing a prostate biopsy. Hot off the press in @TheLancetOncol 🎉 In 660 men across Australian sites with PI-RADS 2-3 but high clinical risk, PSMA-PET: - Avoided biopsy in nearly half (49%) - Was non-inferior for detecting clinically significant cancer (12% vs 16%) - Halved the overdiagnosis of insignificant cancer (32% to 14%) Fewer biopsies, less overdiagnosis, whilst not missing clinically significant prostate cancer: a real step forward for the diagnostic pathway. Enormous congratulations to @ButeauJames @PeterMacCC , who led this as the centrepiece of his PhD and presented it as a plenary at #EAU26 in London. And to my brilliant co-lead @drlouiseemmett - this was a true partnership across our two centres. Thank you to the UROLOGISTS @DrDanielMoon @declangmurphy (& many others), nuclear medicine physicians, technologists, radiopharmaceutical scientists across every site, and to @AnnetteVDHeyden and the @pros_tic who held it all together. We are grateful to our funders: @PCF_Science, @nhmrc, St Vincent's Curran Foundation, Peter MacCallum Cancer Foundation, and @ANZUPtrials Most of all, thank you to the 660 men who took part. Congratulations to Dr. Helen Nadel @pednucrad on delivering the Peter Valk Memorial Lecture at the @SNM_MI Annual Meeting today - a truly well-deserved honour for a remarkable career. The Award honours the legacy of Dr. Peter Valk, a pioneer in establishing PET as a cornerstone of clinical medicine. Her lecture title - "Dots, Tots, and Docs: A History of Pediatric PET Imaging" - beautifully captures her lifelong commitment to the smallest patients and the big science that serves them. 💙 #SNMMI26

Lutetium-177 PSMA RLT has shifted the paradigm in mCRPC, but we always need a reality check 🔬 The Good: Unparalleled theranostic precision: tumour shrinkage, major Quality of Life improvements 🛑 The Catch: Disease recurrence remains virtually inevitable. Why? It struggles to suppress micrometastatic or heterogeneous disease. 🔮 The Future: Moving to earlier-line therapy, adaptive dosing, & next-gen alpha or Auger-electron emitters (like Terbium-161 and Lead-212). The goal? Turning favourable initial responses into durable cures. Full insights just published in European Urology Focus 👇 authors.elsevier.com/a/1nBKS…

The last 2 days at IPCS 2026 @APCCC_Lugano have been a masterclass in translational research in Prostate Ca. Enjoyed the conversations in radiopharmaceutical therapies which have clearly shifted from "Does it work?" to "How do we make it work better for the individual?" Some personal highlights: Dose Intensification: @drlouiseemmett 's presentation of the OPTIMAL-PSMA trial results using Lu-177 PSMA-597 was a standout. We’ve seen the potential of RLT, but the move toward dose intensification (and a quite heavy one at that) driven by personalized dosimetry is how we are trying to close the gap for patients with higher disease burdens without sacrificing safety. The New Radionuclides: @DrMHofman gave a breakdown of the current and emerging radionuclides including Tb-161 and Pb-212. The beta/Auger emission of Terbium-161 and the high-LET alpha emissions from Pb-212 are practical tools to address micro-metastatic disease and the resistance we often see with standard beta emitters. Expanding the Target List: Ken Hermann discussed the role of KLK2 and provided a nice overview of other biomarkers (both that may work as well as those that may not!). As we know, PSMA-negative or heterogeneous disease remains a major clinical hurdle. Seeing the emerging data on these additional targets gives us a roadmap for biomarker-driven selection. The common thread in these discussion was our gradual move towards a highly nuanced, imaging and blood biomarker-driven, combination-therapy model where the radionuclide and the target are selected based on the specific biology of that patient’s cancer. It’s an exciting time to be in Nuclear Medicine! Thanks to @Silke_Gillessen Andrea Alimonti for organizing IPCS-26 as the excellent translational companion to the APCCC.

Early morning "run" (mostly a scenic walk) by Lake Lugano. 🇨🇭 Passed by the Palazzo dei Congressi, ready for @APCCC_Lugano 2026 and #IPCS2026. Fun fact: While we’re here for prostate cancer, this city is the birthplace of the Lugano Criteria for lymphoma! #NuclearMedicine #Radiology #MedEd
