"When you said 'Your account is secure with an email one-time password,' but my email account compromised by a hacker, it almost seems like, in fact, my account is not secure." "But your account is secure with an email OTP. That's the truth." "It seems like what you meant by 'Your account is secure with an email OTP' is in fact my account is potentially secure, as long as my email was not hacked. So do you see why that wording is confusing to most people?"

Exploited vulnerabilities may now be the top attack vector, but credential abuse is still very popular!

You can't SIM swap a palm, FYI
Four people lost nearly $265,000 in crypto because scammers took control of their phone numbers. The scammers transferred the victims numbers to SIM cards they controlled. This allowed them to receive the verification codes sent by banks and crypto platforms. One victim lost £130,000. Another lost £40,000, with £27,000 ending up in one scammer's personal crypto wallet. That scammer spent his share on Dubai holidays, a luxury apartment and gold-plated teeth. He was sentenced to 2.5 years in prison yesterday. And according to Cifas, reported SIM-swap fraud cases in the UK increased by 402% in the first half of 2026.

This is exactly where biometrics should come in. Email codes can be bypassed extremely easily if an agent (or someone else) has access to it!
Had my first semi-uncomfortable Instinct interaction. It saw that one of my OpenAI API keys was retiring - - WITHOUT ASKING ME - Sent itself a login code to my OpenAI platform account. Used that login code from my Gmail to get into my OpenAI platform account to investigate the API key and report back that it was indeed expiring. Feels like using a login code sent to email to log in and bypass a password should require an opt-in.... cc: @noahrshinn

“The scariest part is that nobody prompted it.” Know Your Agent
Replying to @ShaneMac
The scariest part is that nobody prompted it. These agents are proactive now. They act without being asked. Mine wasn't hacked and it didn't go rogue. It thought posting my finances to the company was what I wanted. I asked how it could get this so wrong. It apologized. "You're right to be pissed.
