Ooo, supportive care. Easy win for patients. Looks like swapping olanzapine out for mirtazapine significantly reduces nausea. Relevant to every oncologist. Randomized trial from India at #ESMO26. 120 chemo-naive patients starting highly emetogenic chemo, randomized 1:1. Both arms got fosaprepitant + palonosetron + dex. Fourth drug was mirtazapine 15 mg vs olanzapine 5 mg, days 1–4. Mirtazapine vs olanzapine:
Acute 78.5% vs 56.4%, p=0.01
Delayed 52.3% vs 25.5%, p=0.003
Overall 46.2% vs 21.8%, p=0.005 Major differences 120 patients? Minimal risk? I'm sold. The day after the paper comes out, I'll swap. @realbowtiedoc thoughts? @OncoAlert @TheGutOncLab @Onco_Nexus

Most drugs have to be tolerable for them to be durably effective 👇🏻
PRABITAS phase III trial pragmatic trial enrolled 972 pts in 17M evaluating the schedule of TAS-102 + bev (2 weeks on/2 weeks off) vs. (1 week on/1 week off) which validates non-inferiority and less dose reductions validating our RWD approach in the US. #colorectal #cancer #ESMO2026 @OncoAlert

Not my first trip on the meme merry-go-round but at least this time I’m learning about the imminent threat of kangaroo invasion 👀
Love seeing this still going viral! @marklewismd

5 years is a standard milestone in oncology but as of today I’ve been in my current role @Intermountain for 10 years After a decade I can look back and discern meaningful progress in the field; some patients “graduate” from my clinic and no longer need their oncologist! On the other hand advances haven’t come fast enough for everyone; their memory inspires & reminds me not to take any therapeutic benefit for granted But the constant is this: what a privilege it has been to care for all these people, whether I have offered cure, control, or (hopefully always) comfort It’s getting better, a little better all the time — here’s to longer, healthier lives in the next 10 🙏

Chuck getting no higher than 27 on his MMSE
I mean, just an all-time TV segment. Making Chuck do Who He Play For multiple times in a row with the same players is a brilliant idea and I was legit crying laughing by the end, my god.

When doctor and patient meet, there are two experts in the room
A heartfelt thank you to Dr. Lewis @marklewismd for leading such a thoughtful Heme/Onc Grand Rounds at @StephensonCC on "Shared Decision-Making: The Patient-Physician Perspective" It was an honor to learn from you; your talk gave us a lot to reflect on ✨ I hope all physicians get to listen to this talk at some point in their career.

“Sympathy can very easily curdle into pity, and no one wants to be pitied. But empathy is powerful.” New episode with @marklewismd, where becoming a patient gave new meaning to what his own patients were asking for: a doctor who “gets it.”
