ARM AND LEG PAIN IN 77M WITH MULTIPLE MYELOMA
HPI: The patient is a 77-year-old male who presents to the emergency department following a ground-level fall. He reports right upper extremity pain that preceded the fall, as well as significant left thigh pain with weight-bearing. He was recently diagnosed with multiple myeloma and is awaiting initiation of treatment. He denies additional pain in the remaining upper and lower extremities.
PMH: Past medical history is significant for multiple myeloma, chronic obstructive pulmonary disease (COPD), and prior right reverse total shoulder arthroplasty (rTSA).
PE: Examination of the right upper extremity demonstrates a well-healed surgical scar and tenderness to palpation over the proximal arm. The neurovascular examination is intact. Examination of the left lower extremity reveals tenderness to palpation over the mid-thigh, with pain elicited during log roll and axial loading.
