This indeed looks too much for valve preservation. Apart from the number, they are large. 0ne or two fenestrations can be managed with good prognosis

significant amount of cusp fenestrations involving all leaflets and mostly pronounced at the free leaflet edges (as expected) was encountered.
How much of leaflet disease is acceptable for VSRR?
