Among the many things I admire about my good friend is his confidence in being himself.
He reminds us that it’s not what you wear, but who wears it, and that intellect, character, and leadership matter far more than traditional expectations of conference attire.

IMmotion 151 clusters are often cited using various and sometimes inaccurate methods. This manuscript describes an individual sample model to be applied moving forward.
Several groups have attempted to predict the IM151 RCC clusters with flawed methods. We developed IRIS-RCC (Individual RNA-seq Intrinsic Subtyping for RCC; available at irisrcc.vindhyadatascience.c…), to predict clusters using an individual-sample model.

ALT Development and validation of IRIS-RCC for predicting molecular subgroups in RCC
A. Conceptual figure showing the modeling used to derive the IRIS-RCC predictor
B. Cross-validation accuracy metrics for the IRIS-RCC predictor, showing boxplots for overall accuracy and sensitivity, positive predictive value, hazard ratio, and odds ratio for each of the cluster groups.
C. Prevalence of the different clusters in the training data (IM151), and validation sets (JR101) using two different methods (Saliby and IRIS-RCC).
D. Violin and boxplots showing the enrichment of PD-L1+ IHC levels as a fraction of the total immune cells in Cluster IO compared to Cluster Exclude and Cluster Angio
E. Violin and boxplots showing the enrichment of CD8+ IHC levels in Cluster IO compared to Cluster Exclude and Cluster Angio
F. Heatmap showing mutational associations with clusters

Excellent presentation by @tompowles1! VOLGA: perioperative durvalumab + 3 cycles of neoadjuvant EV improved EFS (HR 0.51) and OS (HR 0.58) vs cystectomy ± adjuvant therapy in cisplatin-ineligible/declining MIBC. ADCs moving earlier! #ESMO26@oncoalert@myESMO
#ESMO26 Phase II sac-TMT + pembro in 1L cisplatin-ineligible advanced UC (n=40, 60% UTUC)
Median PFS 11.2 mo, OS NR and G3/4 TRAEs 63%
Promising results although its place in the EVP era remains to be defined. Worth exploring this combination in other settings
@OncoAlert
SAC-TMT + pembro shows 68% response rates in metastastic bladder cancer #ESMO2026 . Trofuse-55 explores this combination in bladder sparing approaches in a randomized phase 3. The triplet with EVP seems a justified research randomised question. Triplet data from yesterday showed 81% RRs. @OncoAlert
Kudos @tompowles1 on a fantastic #VOLGA presidential @ #ESMO26 👏
Less is More!
• Cis-inelig MIBC, 25% were cis refusers • D+EV x3 → RC → adj D x9 (NO adj EV) • EFS HR 0.51 • OS HR 0.58 • pCR 45 vs 10% • G3-4 TRAE 30%, • neuropathy only 18% • imAEs 33% (G3-4 11%) • GHS/QoL maintained post-RC
• Treme ↑tox (G3-4 42%, 5 deaths), NO OS gain.
My take: it's not just about chasing bigger numbers. Tox matters too, especially when OS is achieved. Need to discuss the pros and cons with patients.
Less PN and skin tox, and fewer G3+ immune AEs than more cycles and that's a win for patients ! 🙌

VOLGA also explored short periods of EV with duvalumab and tremelimumab in muscle invasive bladder cancer #ESMO26@NEJM. Results showed EVDT improved EFS vs cystectomy (HR 0.61). Significant OS was not reached at this interim (HR 0.77 -ongoing analysis). EVDT = 10% higher rates of G3/4 toxicity and less EV delivery than the EV durvalumab arm. More is not always better. It appears the addition of treme partly compromised delivery of EV, which likely accounts for the more modest results compared to the doublet @OncoAlert
How are new data reshaping the muscle-invasive bladder cancer landscape?
