If/when VOLGA regimen is approved, is this something (3 cycles of EV-Durva) you’d extrapolate for cis eligible MIBC patients as well and use this approach broadly or stick with EV-Pembro (EV304: and push for 9 cycles of EV)? @PGrivasMDPhD @shilpaonc @drenriquegrande @DrKarineTawagi @UroDocAsh @bergsa83 @MikeSerzanMD @BrendanGuercio #ESMO26 @tompowles1 @dr_yakupergun @DrYukselUrun

🚨 One of the most attractive aspects of VOLGA: EV only neoadjuvantly! 100% agree with you Tom @tompowles1 Just 3 cycles of EV + perioperative durvalumab delivered impressive efficacy in cisplatin-ineligible MIBC: 🔹 EFS HR 0.51 🔹 OS HR 0.58 🔹 pCR 45% vs 10% No adjuvant EV required by the regimen. This is particularly relevant because cumulative EV exposure matters: peripheral neuropathy often emerges later during treatment. Incidence 36% in 905/B15 compared to only 18% in VOLGA! Could avoiding adjuvant EV reduce long-term toxicity, treatment burden and financial toxicity while maintaining excellent outcomes? Importantly, VOLGA cannot tell us whether omitting adjuvant EV is equally effective as the longer EV regimens used in KEYNOTE-905/B15. Cross-trial toxicity comparisons are also confounded by different populations and reporting. The next step should be response-adapted perioperative treatment, potentially integrating pCR and ctDNA clearance to identify who truly needs postoperative intensification. Maximize cure. Minimize unnecessary treatment. 👏 #ESMO26 #BladderCancer @OncoAlert @weoncologists @urotoday @Uromigos @OncoAlert and Faculty: @DrChoueiri @apolo_andrea @neerajaiims @montypal @Silke_Gillessen @AOmlin @brian_rini @PGrivasMDPhD @TiansterZhang @amerseburger @DrDanielHeng @HHammersMD @scocmem @AndreaNecchi @AmandaNizamMD @drenriquegrande @nataliagandur @AlbigesL @cdanicas @declangmurphy @seanmmcbride @shilpaonc @DrRanaMcKay @scserendipity1 @DrYukselUrun @ravikanesvaran @BraunMDPhD @EfstathiouEleni @mzugman @crisbergerot @Markuseckstein3
Why ‘More Drugs’ Isn’t Always Better: VOLGA Trial Redefines #BladderCancer Care #Podium2Screen from the #ESMO26 Presidential Session Presented by our @OncoALert 🚨faculty @tompowles1 Perioperative Durvalumab ± Tremelimumab Plus Neoadjuvant Enfortumab Vedotin in Muscle-Invasive Bladder Cancer: Results From the Phase 3 VOLGA Trial @jteoh_hk @CPRT65 🔬 Phase 3 VOLGA trial (n=695) Evaluated perioperative durvalumab (D) ± tremelimumab (T) plus neoadjuvant enfortumab vedotin (EV) versus control (radical cystectomy [RC], followed by observation or approved adjuvant therapy) in cisplatin-ineligible muscle-invasive bladder cancer (MIBC). Just PUBLISHED in @NEJM nejm.org/doi/full/10.1056/NE… Interim analysis; median event-free survival (EFS) follow-up: 22.9 months. ➡️ D + EV: EFS HR 0.51 (95% CI, 0.38–0.67); OS HR 0.58 (95% CI, 0.40–0.83) ➡️ D + T + EV: EFS HR 0.61 (95% CI, 0.47–0.80); OS did not meet statistical significance ➡️ Pathological complete response (pCR): 45% with D + EV vs 10% with control ➡️ Grade 3/4 treatment-related adverse events (TRAEs): 30% with D + EV vs 42% with control ✅ Both primary EFS endpoints were met. Triplets in 1st line RCC failed in LS12 @Annals_Oncology @brian_rini (n=1500!). There are not many combinations with HIF/VEGF/PD1 left to test and this approach has probably run its course. It’s time for a new chapter in RCC with T cell engagers and bispecifics #ESMO26 @montypal @DrChoueiri @OncoAlert

Several groups have attempted to predict the IM151 RCC clusters with flawed methods. We developed IRIS-RCC (Individual RNA-seq Intrinsic Subtyping for RCC; available at irisrcc.vindhyadatascience.c…), to predict clusters using an individual-sample model. nature.com/articles/s41698-0…

The MOIO randomised phase 3 non-inferiority trial (n=340) explored early discontinuation of ICI therapy be SOC in advanced patients responding to therapy. It had many flaws (mixed tumors and treatments, small size, early discontinuation etc). Also, due to the statistics, it’s not possible to say whether non-inferiority was achieved (OS HR 1.89 [95%Cl: 1.03,3.48]). Despite these problems it highlights a concern around early discontinuation, which should be taken seriously. Tumour specific trials are needed, but I’m not sure it will be easy to randomize patients . #ESMO26 @OncoAlert @myESMO @Annals_Oncology

Phase 1b combination data for elfetabart drozuntecan (B7H3/Topo-1 ADC) combined with pumitamig (VEGF/PD1 bispecific) shows striking response relates with ~40% G3/4 AEs. Perhaps the best new combination data #ESMO26 . EV + VEGF/PD1 combos are under investigation in bladder cancer too with EV. @OncoAlert
