🚨 VOLGA: chemo-free durvalumab + enfortumab vedotin beat surgery alone in muscle-invasive #bladdercancer 🚨 @NEJM today 🧵1/2 👥 n=695, cisplatin-ineligible or declined MIBC 📊 Phase 3 RCT, 1:1:1 💊 Perioperative durva + EV vs durva + tremelimumab + EV vs radical cystectomy alone ⏱️ Estimates at 24 mo ✅ Durva + EV EFS 63.5% vs 45.1%, HR 0.51 (0.38-0.67), p<0.001 ✅ OS 78.4% vs 69.4%, HR 0.58 (0.40-0.83), p=0.003 ✅ Durva + treme + EV EFS HR 0.61 (0.47-0.80), but grade ≥3 AEs 79.5% vs 71.1% ⚠️ The 2 durvalumab arms were not compared head-to-head, so the CTLA-4 add is not clearly worth the added toxicity 🎯 Another cisplatin-free ADC + IO backbone for cisplatin-ineligible MIBC

Many of my followers are urologists, and I'm talking with some of them this week, so I thought they might appreciate one of my early 🧵on the novel ways that Foley catheters have been used *other than* for draining the bladder. 🧐

With all the discussion about radiation as a gold standard for intermediate-risk prostate cancer, I have one request: Please do not radiate a patient with a peak flow <10 mL/s or PVR >150 mL without having them evaluated by a non-oncologic urologist first. Multiple studies suggest a benefit to addressing significant bladder outlet obstruction before radiation. Treating obstruction afterward can be far more complicated—and sometimes you’ve lost the best window to intervene.

Oral boards prep hasn't appreciably changed in decades, so I built MockUro, an AI examiner that walks you through ~250 canonical urology cases in Socratic Q&A format, grades your free-form answers across key domains, and adapts your study plan to your weak spots 🧵

Reconstructive urology is broad. A @SocietyGURS fellowship built on urethroplasty and AUS is a different year from one built on gender-affirming surgery or ureteral reconstruction. A resident asked me which program focuses on what. I didn't have a good answer. This is my attempt at one.

For those doubting that the Modifier 25 fight will affect urology, look at the ACS data. Urology is one of the most affected specialties: ~597,000 urologic procedures were performed with a same-day E/M. Common affected codes include cystoscopy, bladder irrigation/instillation and Foley placement. Can anyone say with a straight face that placing a Foley means you did 50% less E/M work? Why should putting in a catheter diminish the separately identifiable evaluation you performed? Yet the AUA has still not publicly taken a stance. Regardless, urologists can speak for themselves. Submit your own comments to CMS by Sept. 14. facs.org/advocacy/health-pol… @AmerUrological @CMSGov

Important change in the 2026 @AmerUrological NMIBC Guidelines: all high-grade Ta tumors are now classified as high-risk. This reduces a long-standing source of heterogeneity in the intermediate-risk group and should improve risk-appropriate treatment for patients, while creating cleaner, more meaningful populations for clinical trials. Great to see the evidence translate into practice. @IBCG_BladderCA @UrogerliMD @pjhensley11 @AmirHorowitz @mouwlab @spsutkaMD @shilpaonc @AndreaNecchi @LAUrology_NL @karima_oualla @PGrivasMDPhD @marigfern @SpiessPhilippe @UroCancerMD
Time to revisit something I’ve been saying for years: TaHG bladder cancer is not intermediate risk. The clinical outcomes, molecular biology ... all point in the same direction. Yet some frameworks allow < 3cm TaHG tumors to be downgraded into “intermediate risk” alongside low-grade disease. That classification has real consequences for patients: • undertreatment • less rigorous surveillance • in some studies, these patients are even placed on observation as a supposedly appropriate “control arm.” TaHG is high risk. 🧵 @BladderCancerUS @WorldBladderCan @IBCG_BladderCA
