There are limited prospective data defining the optimal management of intraprostatic radiorecurrence (IPR) after definitive RT for prostate cancer.
We asked:
1. Is dose-escalated focal salvage high dose rate brachytherapy safe and effective for IPR?
2. Can Decipher (@Veracyte) identify patients with higher risk of recurrence with salvage ReRT that may benefit from therapeutic intensification?

In this trial of RT+ADT for int/high risk prostate cancer, substituting a 5ARI (finasteride) for LHRH agonist (Lupron) improved 6 mo hormonal QOL. With longer follow-up, the oral only group of 5ARI + 4 mo bicalutamide had high control rates and improved lab values (chol and hg)..
