Takes One to Know One! Excellent hack from @jbspadoni to distinguish wire-induced spasm from dissection or geographic miss. 👌🏼😀 The real wonder is not that it works, but why? 👉🏼 The caravel has a hollow core and braided shaft, matching the coronary shape once the wire is removed. 👉🏼 Wire shafts, however, are stiff despite being thinner—thus more likely to induce spasm, even with a soft tip.
Managing tortuosify can be trick: Large guide for support Micro options (small with conform, large will straighten) Caravel without wire to sort pseudo from dissection DCB for the win

Learning to Take the Path Less Traveled Three of the best cases at @ctoessentials 2026 involved epicardial collateral crossing by the best in the business: @DrBIqba, @RinfretStephane, and Alex Nap. I applied those lessons as soon as I returned to Houston! Here is the case. 😀

I did my first retrograde CTO PCI in 2009 with my close friend @RumoJose Since then I have performed hundreds of CTO cases, usually challenging and rewarding, but sometimes frustrating too… But I still enjoy facing every CTO and sharing tips and tricks with my team 🙂

An incredible number of trials published simultaneously in the NEJM: I honestly can’t quite wrap my head around what is happening. Congratulations to everyone involved. It was difficult to choose only five trials, but these are the ones I selected for the interventional cardiology highlights. #ESCCongress

6/6 The take-home for complex PCI: Protection is not immunity. Mechanical support may preserve perfusion and reduce the need for rescue pharmacology, but it does not eliminate the myocardial consequences of ischemia. LOPP should not simply be interpreted as “device failure”—it may be a marker of severe ischemic intolerance and ventricular stunning. And perhaps the most important question is not: “How many rounds can we safely fight?” …but: “When are additional rounds no longer worth fighting?”

🧵 New @JACCJournals study from #CHIP-BCIS3 provides an important physiologic look at #Impella-supported high-risk #PCI—and challenges how we think about “protected PCI.” The key message: hemodynamic support ≠ myocardial protection. 1/6

Congratulations to @Bernard_TW_Wong et al for this excellent practical paper on active SB protection in provisional stenting with case examples, IVI images and illustrative descriptions. Thanks for including me. Lets simplify our learning to improve outcomes for patients
Our group published an updated #review of jailed techniques in @AsiaInterv, an enhanced method of treating bifurcation lesions with a #provisional approach. It really helps to simplify procedures, minimizing the risk of side branch #compromise during main vessel #stenting. ☺️

Here is an older paper with some nice IVUS images. You are obviously very experienced at interpreting IVUS runs. This is a nice atlas for our colleagues who may be ‘re’-discovering the joys of IVUS! 😀🙏🏼

Or, is this a vein as suggested by @nyalborgesmd? IME, veins run across from one side of the circumference to the other. This space appears and lingers in the same position (6 to 8 o’ clock) before disappearing. Also, it has tapered edges (like a dissection), and it obscures the EEL. I think this may be deep injury to the adventitia from severe spasm. Not sure. But the pt. had bonafide STE and made some enzymes. I would wager the endothelium is abnormal in this segment - and in fact appears to be denuded in places.
